Formative Internal Quality Assurance

hese instructions detail the steps to follow when performing Formative Internal Verification (“IQA”) ‐ such as sampling assessment practice.

1. From the Candidate list select the Candidate’s Portfolio you wish to IV by clicking the 'View Portfolio' button.

2. When viewing the Candidate’s Portfolio Front Page, select the green "Evidence" tab on the right.

You will now be viewing a table listing all the evidence in the Candidate’s portfolio.

3. Click on the Evidence you wish to IV and click 'Verify', this makes you the holder of the page and will add this item to your task list (the evidence status must be Assessment complete or Action Required).

You can now also read the Primary Assessor Feedback and any second‐line assessment or previous IQA feedback.

The Plan can be viewed by clicking the 'Show Plan' button at the bottom of the page. The 'Go to Plan' button will navigate you to the Plan, and away from the Evidence.

Note: If there are no 'Plan' buttons, then this would indicate the evidence is not linked to a plan. You can check by clicking 'Show Details' at the top of the screen, and the 'Plan Created Date' will display as (None).

4. Once you are satisfied that they have conducted a sufficient sample and you are ready to provide feedback, click the 'Prepare Feedback' button.

You will now be able to type your feedback and make your IQA decision. If action is required you will need to set a target date. On this page you can also indicate which standards you sampled by ticking the check box provided next to each of the standards which have been linked to the evidence.

When preparing IV feedback, the the following options are available:

Positive decisions: these will lock the evidence and prevent further assessments (subject to course settings)

Agree
Not Sampled

Negative decisions: 

Assessor to Action - On accepting the IQA feedback the Evidence status will move to "Assessing", and the new assessment feedback will be created.  All previous feedback records will remain.
Assessor to assess again - On accepting the IQA feedback the Evidence status will move to "Assessing", and the new assessment feedback will be created. All previous feedback records will remain.
Assessor to revise assessment feedback - On accepting feedback the Evidence status will move to "Assessing", and the Assessor will be able to change the latest assessment feedback record.
Assessor to revise evidence - On accepting the IQA feedback the Evidence status will move to "Revising" and the Assessor will be able to edit the Evidence. This can be used to remove invalid attachments or remove confidential information from Evidence without sending it back to the Candidate first.

5. Click the 'Save Feedback and Close'  (or 'Finish Verification' - only available to qualified IQAs) button, the next step to follow will depend on whether you are a qualified or unqualified Internal Quality Assurer:

Qualified Internal Quality Assurer:  Click 'Finish Verification' ‐ your feedback will now go to the relevant Assessor for them to read and accept.
If you Agree with the assessment decision the Evidence status will read "Verified"; if there is Action Required (see 'Show Help' on page for further details on options) the Evidence status will read "SLA or IQA Feedback Given"; and when the Assessor accepts your feedback they will be able to modify their assessment and/or modify the links again.

Un-Qualified Internal Quality Assurer: Your feedback will need to be countersigned before it goes back to the Assessor.  Use the 'Send' control, at the bottom of the Evidence to send your feedback to a qualified IQA to counter-sign.
If the qualified IQA agrees with your feedback your IQA feedback will now go to the relevant Assessor for them to read and accept. If you Agree with the assessment decision the Evidence status will read ‘Verified"; if there is Action Required the Evidence status will read "SLA or IQA Feedback Given"; and when the Assessor accepts your feedback they will be able to modify their assessment and/or modify the links again.

5.2  Counter-signing/Secondline IQA

5.2.1 Click on the 'Second Line Verifying' Task on your dashboard
5.2.2 You will now be viewing the evidence which has been verified by the unqualified IQA
5.2.3 You can now click:

Agree: this will move the workflow forward so that he assessor can read and accept the IQA feedback
Disagree: Moves the workflow back to step 3 above, for the unqualified IQA to revise their feedback.

5.2.4 Use one of the following features to feedback to the Unqualified IQA:

Unit Comments

Note: Once the Assessor has responded to the IQA feedback a ‘task’ item will appear in your task list titled ‘Last IQA feedback was Disagreement’ you can then repeat steps 3‐5 above.  This task will provide you with a convenient link so you can IQA the Evidence to check that the action has been resolved. If the Assessor fails to address the feedback before the target date, you will receive a task item titled ‘Overdue second line assessment or IQA action’ ‐ this will again provide you with a convenient link to the relevant Evidence for you to monitor.
